Scanned Copies Of the following:
- ‘O’ level result
- Birth Certificate/ Sworn Affidavit.
- Evidence of purchase of Admission form.
- One (1) passport photograph.
- Change of Name Document-where applicable(Marriage Certificate/ Newspaper Publication).
