Federal University Lafia, FULAFIA medical fitness form for the 2017/2018 academic session newly admitted candidates.

This is to inform all the candidates offered provisional admission into the Federal University Lafia (FULAFIA), Nasarawa State, Degree Programmes for the 2017/2018 academic session, that they are required to download and fill the medical fitness form.

All admitted students are to download the Student Medical Fitness Form from the link below and carry out all the necessary medical examinations and submit same to the Medical Centre, FULAFIA.
