LUTH SHIM Admission Form 2025/2026 | ND & HND

The School of Health Information Management (SHIM) at the Lagos University Teaching Hospital (LUTH) has started admission for its 2025/2026 academic session. You can now apply for the National Diploma (ND) or Higher National Diploma (HND) in Health Information Management.

This school is fully approved by the National Board for Technical Education (NBTE) and the Health Records Officers Registration Board of Nigeria (HRORBN).

Meet the ND admission requirements for SHIM

To apply for the ND programme, you must have:

• Five O’level credits in no more than two sittings, including:
  • English Language
  • Mathematics
  • Biology
  • Chemistry
  • Physics
• A JAMB score of 150 or above
• A pass in the POST UTME exam held by SHIM

You must apply with valid details and attend the school’s screening exercise.

Meet the HND admission requirements for SHIM

To qualify for the HND programme, you need:

• The same five science subject credits as the ND
• A National Diploma in Health Information Management from an NBTE-accredited school, with at least a Lower Credit
• One year of work experience (industrial or clinical attachment) at a recognised health facility
• A valid email address for all application messages
• To sit for both a written test and an oral interview at the SHIM school hall or Human Resources Centre

Follow these steps to apply for LUTH SHIM admission

Applying is simple and done online. Follow this process:

1. Visit the portal: luth.gov.ng/schools
2. Choose SHIM as your preferred school
3. Fill in the Account Creation Form with correct personal details
4. After account setup, check your email for your login details
5. Pay the ₦10,000 application fee via the Remita platform (you may pay immediately or later)
6. Log in at luth.gov.ng/profile and complete the application form
7. Print your application bio-data from your email – this will be required at the entrance exam
