Here's a step-by-step guide to get the form:

Step 1 – Make Payment

Pay a non-refundable fee of ₦25,000 to either of the accounts below:

Option A – EcoBank

Option B – Akwuchukwu Microfinance Bank

Step 2 – Upload Payment Details

After payment, upload your teller details to:

Make sure your email address is correct – your PIN will be sent there.

Step 3 – Fill and Submit Application Form

Once you get the PIN via email:

Your slip has all exam details and must be brought to the exam hall.
