FUOTUOKE Direct Entry Screening Form

The Federal University Otuoke (FUOTUOKE) Direct Entry Admission Screening Form for the 2024/2025 academic session is now available.

This is an opportunity for JAMB Direct Entry candidates to apply for admission into FUOTUOKE.

Eligibility for FUOTUOKE Direct Entry Admission

To be eligible for the FUOTUOKE Direct Entry admission, you must meet the following requirements:

1. First Choice Selection: You must have selected FUOTUOKE as your first choice in the 2024/2025 JAMB Direct Entry.
2. Academic Qualifications: You must hold any of the following qualifications: NCE, OND, HND, First Degree, or A’ Level in relevant disciplines and subjects. Additionally, you must meet the O’Level requirements for the relevant programme.
3. Change of Institution: If you did not initially choose FUOTUOKE but wish to study there, you must change your institution to FUOTUOKE via the JAMB website and select one of the available programmes.

How to Apply for FUOTUOKE Direct Entry Admission

Follow these steps to apply for the FUOTUOKE Direct Entry Admission:

1. Payment: Make a non-refundable payment of ₦2,000.00 using an Interswitch Verve or Mastercard debit card.
2. Online Application: Visit the FUOTUOKE application portal to complete your online application.

Documents Required for FUOTUOKE Direct Entry Admission

You need to upload the following documents during your application:

• A digital passport-size photograph.
• A scanned copy of your WAEC, NECO, or NABTEB O’Level result (in PDF format).
• A scanned copy of your LGA identification letter (in PDF format).
• A scanned copy of your Birth Certificate or Age Declaration (in PDF format).
• A scanned copy of your NCE/OND/HND/First Degree or A’ Level certificate (in PDF format).
• A scanned copy of your academic transcript (in PDF format).

Special Instructions

1. Ensure your digital passport photograph is in JPG format.
2. Provide a valid email address and personal phone number during your application.

Note: FUOTUOKE will not be responsible for unsuccessful applications due to invalid email addresses or phone numbers.

Application Timeline

• Commencement: The online application begins on Monday, 19th August 2024.
• Deadline: The deadline for submitting your application is 8th September 2024.

Important Notices

Candidates who fail to upload their academic records on JAMB CAPS or make false declarations regarding their O’Level result grades or state of origin will not be considered for admission.

Direct Entry Screening Exercise

The date for the Direct Entry Screening Exercise will be announced after the completion of the application process. The schedule will be available on the Direct Entry Screening Schedule Printout.
