10. Original Medical Certificate
11. Photocopy of Certificate of State of Origin
12. Photocopy of Birth Certificate or Declaration of Age
13. Two copies of School Fees Breakdown & Acknowledgement Slip
14. Photocopies of RRR & Acknowledgement Slips for All Payments
15. Photocopy of Oath Form
16. Two Current Passport Photographs

11. Academic Transcripts
12. Original IT Letter, duly signed & stamped
13. Photocopy of School Fees Breakdown
14. RRR & Acknowledgement slips for all payment

16. JAMB Regularization Slip/Photocopy of JAMB Admission Letter (Institution copy)
17. Two Current Passport Photographs
